All students moving into Year 11 must have a USI number so that all VET education and training as well as any courses completed whilst at school is recorded through you USI number that you will use to pursue further education and training when you leave school.
Step 1: Fill in your details on the form using a Medicare card. Be sure you fill in all details correctly and you can also take a photo of the card to ensure that the process can be completed at school.
Step 2: Fill in the Privacy Notice with your signature and a parent signature. Hand this in to your teacher.
Step 3: Follow the instructions on completed sheet 1 and create your USI using the website www.usi.gov.au
Step 4: Write the USI number on the sheet and hand to your teacher.
